Faith Family Medical Center

MISSION: Driven by faith, we provide hope and healing to our hardworking neighbors in need. The purpose of Faith Family Medical Center is to provide the residents of greater Nashville who do not have health insurance with affordable, quality primary medical care. Faith Family seeks to become the family physician for these residents. With a low-cost primary care provider available to them, people will be able to seek out medical care at the onset of any problems. Treatment can then begin early on instead of after their conditions have become more complicated. Clinic patients also benefit from counseling and education services designed to help them achieve a healthier lifestyle.

IMPORTANT UPDATES TO OUR SERVICES AND PATIENT QUALIFICATIONS:

Telehealth Services

Faith Family continues to implement new solutions to improve care and convenience. We have implemented telehealth services which are available at no cost for existing patients for the safety and well-being of our employees and patients. Types of telehealth appointments include diabetes, hypertension, cholesterol, ADD/ADHD, depression, anxiety, and medication refills. Providers perform all parts of a normal office visit, except for the physical exam, and patients can discuss their medical needs and concerns from the comfort of their own home.

Expanded Patient Qualifications

We remain committed to supporting the needs of our community in the face of recent unprecedented challenges. Our Executive Committee has approved an interim policy until July 1 that will allow unemployed persons to receive care at FFMC. Patients must show proof of previous employment within the past 6 months and must secure employment within the next 6 months if continuing care is provided by FFMC. Please contact our clinic for any question about eligibility at 615-341-0808.
